Here's how to reserve your places at the Ball.
Here's how it works:
- You decide whether you are purchasing individual admissions or are sponsoring a table.
- You download the appropriate form from this page and complete it.
- The forms are set up so that you can enter the information using your keyboard before printing. You'll need to open the form in Adobe Acrobat Reader to activate the functionality.
- You mail the completed form and your check to the address on the form.
- You will receive an e-mail in confirmation which you can bring with you as proof of payment (we'll also have your name on our roster, so you're covered).
- You arrive and enjoy a festive evening.
